What Is Ghrelin?

Anyone who’s tried fasting knows this one without needing the science, because you feel it. There’s a specific window where hunger goes from nothing to unbearable in what feels like ten minutes flat. That’s ghrelin, and knowing its name changed how I handled it.
The Tantrum
Ghrelin doesn’t rise in a straight line the longer you go without food. It comes in waves, and there’s a specific wave, roughly hour sixteen to twenty for most people, that hits hard and loud, like a toddler having a proper tantrum in the middle of an Aldi aisle. Full volume, seems endless, deeply uncomfortable to sit through. And then, like most tantrums, it passes. Ghrelin drops back down whether you feed it or not. Knowing that in advance is the whole trick, because the tantrum ending isn’t conditional on you giving in to it.
Ghrelin is a hormone produced mainly in the stomach that signals hunger to the brain. Levels rise before typical mealtimes, shaped heavily by habit and routine as much as genuine need, and fall after eating. During fasting, ghrelin rises and falls in waves rather than climbing continuously, with intensity typically easing after the first fasting attempts as the body adjusts to a new eating rhythm.
Research on fasting physiology consistently describes ghrelin as wave-like rather than continuously rising, with hunger commonly reported as easing rather than worsening the longer a fast continues past the initial hours.
